To improve patient experience, making sure we get patients to see the most appropriate clinician in the team referral to the Pain Team is a two part process, linked to the MyHealth – Devon NHS website:
Part One – Healthcare Professional completes
Is the referral from GP, health, social or mental health practitioner. The easiest way for them to do this, is write a referral letter and send this by email via choose and book to the Devon Referral Support Service.
Part 2 – Patient completes
We require some additional information from the patient to help us decide how best to support them. This information is provided through completion of a short questionnaire that is hosted on the My Health website. Further details can be found here:
Once both parts of the referral process are completed one of the clinicians in our team will triage and an appointment will be made. Your initial assessment can be with any member of the team. Who you see will depend on your individual need and the skills and strategies you require to help manage your pain.
