Creating place-based offers of support

The main objectives of this project are to test out whether community and social approaches to supporting people to manage their health are effective and whether they are an efficient use of resources. This will be achieved through:

1. Testing a diabetes wellbeing club model in Fremington, Ilfracombe & Torrington.

The first Wellbeing Club had 20 patients in attendance together with a practice nurse, practice manager, GP, Specialist Diabetes Dietitian, Specialist Diabetes Nurse, podiatrist, health promotion officer, mental health representative, One Small Step officer, pharmacist and fire community safety officer.

2. Testing peer support and expert patient models.

3. As a result of the evaluation of the tests of change, creating a proposal for place-based offers of support to be further tested.
