NHS Continuing Health Care

About us

NHS continuing health care is a package of care that is arranged by and funded by the NHS for eligible patients. The team consists of specialist nurses who assess patients using a national framework to see if they qualify for NHS funded care.

How do I access this service?

To be eligible for NHS continuing health care you must be over 18 and have a complex medical condition and substantial and on-going care needs. If you have been diagnosed with a long-term illness or have a disability this does not mean that you will automatically qualify for NHS continuing health care.
