In line with Northern Devon Healthcare Trust Trust vision and 2017/18 plan and NHS England’s 6 C’s we support independent care providers within North Devon. Our service offers resources, anticipatory contingency planning, guidance & support in addition to the delivery of structured education and training programmes which compliments statutory training requirements.

Providing a free service to: care homes (nursing & residential), domiciliary care providers, supported living, day centres and those living in their own home.

The service we offer is:

  • To upskill staff and empower them to be confident in the delivery of care and support
  • To prevent avoidable hospital admissions for people living in their own home, supported living, residential and nursing care homes within Northern Devon.
  • To safeguard the wellbeing of those in receipt of care
  • To reduce the risk of avoidable harm such as pressure damage, falls and malnutrition
  • To reduce the risk of avoidable acute events such as UTI’s
  • To build upon existing, established and effective partnership working relationships with independent care providers
  • To involve those in receipt of care and their families, promoting self-care and control in addition to helping family to understand care practice & the roles of care staff
  • To support the delivery of excellent standards of high quality care within independent care services across Northern Devon through the provision of support, education and training.
  • To support the delivery of an exceptional client experience within independent care services within Northern Devon
  • To work in partnership with independent care services and multi-agency colleagues in doing so
  • Support and education provided is underpinned by good practice
  • To utilize data to measure improvement
