Self-reporting of adverse reactions to medication

Patients or carers can report suspected adverse drug reactions to the Medicines and Healthcare products Regulatory Agency (MHRA), through the “Yellow Card” Scheme (Reports used to be made by completing reports printed on yellow card).

Adverse reactions to medication can be reported to the MHRA by:

  • Completing an electronic form online: https://yellowcard.mhra.gov.uk/
  • By telephone on Freephone number 0808 100 3352 (10am to 2pm Monday-Friday only).
  • Obtaining and completing a patient Yellow Card from your local community pharmacy or GP surgery.
