For antenatal Sickle Cell Thalassaemia screening, a Family Origin Questionnaire (FOQ) form MUST be completed at booking. If no completed FOQ is submitted with the samples, testing cannot be undertaken. For GP requests, consent must be obtained by speaking to the patient and explaining the nature of the test and implications of the result. | |
