The team behind a project to reduce frequent attender visits to A&E at NDDH has won an award for excellence in integrated working.
The Celebrating Achievement Awards are run by Devon Partnership NHS Trust (DPT), who provide mental health services across Devon. A multi-disciplinary team involving staff from various organisations won the Integration Award for their project.

The project started in 2017, taking an initial group of 12 frequent attenders. All the healthcare professionals involved in their care developed a supportive plan, which was agreed with the attender. This plan was then made available in A&E to be used during any future attendance. Staff can work from the plans and offer immediate options, knowing they have been pre-agreed. This can be extremely helpful in what can be acute/crisis situations.
This work has resulted in a significant reduction in A&E attendance in the first and second cohort of 24 frequent attenders. Outcomes for these individuals have improved as they are managing better and not needing to attend. This has also saved money for the NHS, which can be reinvested elsewhere.
