Service Description: The Macmillan nurse team provides an expert specialist consultancy service to patients, their relatives and those closest to them. We accept referrals for hospital in-patients with life limiting illnesses. We maintain close working links with the local hospices, to provide a seamless service and to promote continuity of care.
The team aims to:-
- Identify and alleviate physical symptoms and to promote patient rehabilitation and well being.
- Facilitate open and honest communication.
- Identify and alleviate social, psychological and spiritual difficulties.
Referrals
Whilst you are in hospital the doctors and nurses looking after you, or your relative, can make a referral to the team.
