The Community Children’s Nursing Team is part of Integrated Children’s Services provided by Northern Devon Healthcare Trust
The Community Children’s Nursing (CCN) Team is a generic community based children’s nursing service consisting of Registered Children’s Nurses; the Team leader who is based in North Devon District Hospital holds a Specialist Community Practitioner Degree in Community Children’s Nursing.
The CCN Team covers all areas of North Devon- Barnstaple, Bideford/Torrington, Ilfracombe/Braunton, South Molton/Chulmleigh and Holsworthy/North Cornwall -provided the child/young person has a North Devon registered GP and/or North Devon registered Paediatric Consultant. Each CCN is allocated to a geographical area where they provide CCN cover. However all team members cross work and cover according to patient/service needs and priority.
Currently the CCN service operates Monday to Friday from 9.00am to 5.00pm. However the CCN’s will work flexibly around these times where possible, to ensure the child/young person’s health care needs are met.
What the CCN Team do
The CCN Team provide specialist and clinical nursing support in the community to sick children and their families and aims to prevent unnecessary hospital admissions, reduce the hospital stay for those children who need hospital care and to prevent referral to secondary services where possible. Care is delivered in a variety of settings to suit the needs of the child and family, e.g. home, schools, nurseries, clinics, and some hospital areas.
How the CCN Team Work
The CCN Team works in partnership and liaises with Primary and Secondary Healthcare Professionals and forges close links with other agencies who may be involved in the child’s care; providing continuing care and support without duplication of roles where possible.
Where possible the CCN team endeavour to empower parent/s, carer/s, children & young people with the necessary knowledge and skills required to carry out their own care with the support of CCN nursing provision through home visits and by telephone support contact.

Referrals to the CCN Team can come from a variety of professionals for example:
Caroline Thorpe Ward and Special Care Baby Unit (SCBU), Consultants, Accident & Emergency Department, General Practitioner’s, Health Visitors, School Nurse, Joint Agency Service
Referrals from any source will be considered. Parents’ consent is needed before a referral can be accepted.
