Service description: The service comprises specialist nursing teams and is in two parts – Cardiac Rehabilitation and Heart Failure Service.
Cardiac rehabilitation provides treatment and rehabilitation (including education and risk factor management, exercise intervention and psychological interventions) for patients with the diagnoses including ST elevation myocardial infarction, Stable & Unstable angina and Non-ST elevation myocardial infarction.
Cardiac failure services should be available in conjunction with the heart failure nurses to ensure patients are on the most appropriate pathway. The Heart Failure Service provides support, education on medication, and symptom recognition and monitoring for patients with heart failure. It also provides training and education programmes for other healthcare professionals.
Service locations: Cardiac rehabilitation is provided in hospital (Phase 1) and in the community (Phases 2, 3 & 4). This service is not currently available to residents of Wenbury, Ivybridge, Yealmpton, Tavistock, Yelverton and Lifton, but development work is underway to extend the service into this area.
Heart Failure clinics are run in many of the community hospitals and nurses also carry out home visits.
