Service Description: The Northern Devon Heart Failure Nurse Service aims to promote high quality care to patients with chronic heart failure, caused by left ventricular systolic dysfunction (reduced pumping function of the main heart chamber). see what is heart failure?
Service Location: We provide services to the wider North Devon community, holding clinics at the North Devon District Hospital, Ilfracombe, Bideford, South Molton and Holsworthy Community Hospitals. We also offer telephone follow up. When patients are unable to attend clinic, we will visit patients at home as appropriate.
