Acute Community Team

What is the acute community team?

This is a short-term service covering 7 days a week for people aged 18+, registered to an Exeter GP who need treatment, care or a short period of rehabilitation. This may be due to a recent acute illness or loss of independence at home.

What is our aim?

Our primary aim is to support you safely in your own home, wherever possible, with health and social care support and to avoid unnecessary and inappropriate admissions to hospital or care home.  We can also help with supported discharges from hospital and have responsibility for recuperation beds.

Who might I see?

Our team includes:

  • Team lead
  • Nurses
  • Physiotherapists
  • Occupational Therapists
  • Generic Therapy Support Workers
  • Co-ordinator

Your GP will be responsible for your medical care.

What do we do?

  • Provide an assessment of your health and social needs
  • Agree what is important to you
  • Make a plan of your needs
  • Work with you to carry out a treatment programme

We may also advise you regarding equipment and care needs.

What else can I expect?

We will discuss your needs with you and your family, taking into account your view and wishes.
